What temperature to store yeast at? (both dry and liquid)

if it was liquid how long was it stored? i keep mine in my small fridge about 35F.
the only yeast so far that went bad was some yeast that i washed and forgot about for a few months. it turned brown, just dumped it and got fresh.
 
What was the date of manufacture and where did you get it? What kind of yeast and what temps did you pitch at?
 
More details are needed to determine what might have happened, but it definitely wasn't killed by cold. Yeast labs keep their cultures under deep freeze: like -80°F.
