How to add lactose and priming sugar?

I have 5 gals of stout that I am ready to bottle. I plan to use 1/2 lb lactose and 3/4 cup corn sugar. It sounds like boiling the lactose for 10 min or so should dissolve and sterilize it, so my questions are:
1) How long should I boil the corn sugar for?
and
2) How much water should I boil?

Any tips or hints appreciated. Thank you!
 
I usually boil 2C of water in a small sauce pan for 10 minutes or so. I then take it off the heat & add the measured amount of priming sugar. It seems to me that lactose is added at the end of the boil?...That's the way I thought I read on here anyway.
