It's for the Pliney the Elder kit from Farmhouse Brewing Supply.
Do I add the first dry hop after 12-14 days, or at the beginning of fermentation?
And second dry hop goes in after 14 days for 5 days?

I'm thinking first dry hop goes in after 14 days for 14 days and second goes in around day 9 after first dry hop.

After your beer is finished primary, you add the Dry Hop CTZ, Centennial and Simcoe, they stay in the secondary (or keg) for 12-14 days. At the 7th day of dry hopping, add the second set of CTZ, Centennial and Simcoe dry hops, remove all the dry hops at the 12-14 day mark, or rack the beer to a serving keg.

I concur, the dry hop schedule isn't coupled to a hard date from the yeast pitch.
That's proper - only an FG check will tell when a brew has reached terminal gravity...

Cheers!
