Boiling the wort (all-grain)

When we speak of boiling the wort for 90 mins, 60 mins, etc., is that timed from when the wort becomes a rolling boil or is it timed from the moment the boil kettle hits the flame?
 
for me (and most home brewers) it is when the boil starts, but whatever you do a make note. that way when you try to make the beer again you will have a better chance at duplicating.

Ok so I assume that means the timing starts when the ROLLING boil starts, not the 40 mins it may take to get it to a rolling boil?
