Amount of Mash Out Water

For those that do a mash out, I'm wondering how much boiling water you add during mash out? On my first all-grain batch, I tried the equation from Palmer's book... amount of water (in qts) = (desired temp-mash temp)(.2*lbs of grain+qts of mash in water)/(water temp-desired temp)

For my batch and assuming roughly 210 degF water temp, I came out with about 5 quarts of water. However, this only got my mash up to around 160 when I was targeting 168. I know I could just heat up plenty of water and add it until I reach my mash out temp. I was just trying to figure out why I didn't have better luck calculating the amount since I have had good luck calculating the temp of my mash in water to reach my target mash temp? Is there something I'm missing in the equation? Thanks for the help!
