Super high mash efficiency?

Designed a recipe based on a clone and getting 75% efficiency. I use Brewology101 for ball-parking my recipes. Here's the pertinent stuff>>

11.5 lb Pale Ale malt
3 lb Vienna
12oz C60L
10oz Aromatic

At 75% efficiency it calculated an OG of 1.075. Well after all said and done I ended up with 5.5 gallons of wort at 1.096.

1.096!!

Please tell me how?! Is Brewology101's "AleAbacus" that off?!
 
Couple of things.....did you measure your grain correctly? Are you measuring your liquid volume correctly? Is your hydrometer calicrated?
 
Used a refractometer AND let a sample cool and measured with a calibrated hydrometer. 15.75 lbs mashed at 154 with 5.5 gallons sparged with 4.5 gal. Boiled 6.6 gallons to about 5.5. I worked at my LHBS so I go in and weigh and mill my own grains. All correct. There's no way I got above 90% efficiency, but that's what my numbers are telling me.
