Need help finding this grains

I'm putting together a wit recipe based upon a list of ingredients a local brewer gave me. I am new at this so please bear with my noob questions.

I need Malted Barley (2-row) but I don't see just a plain malted barely when looking for ingredients at midwest home brewing.

I also need Malted Wheat, flaked unmalted wheat and flaked unmalted barely. Looks like flaked wheat and flaked barley are unmalted already. Not seeing just malted wheat. I do see a pale wheat and a white wheat.

Which grains should I use?
 
White Wheat (Rahr) and Pale wheat are both malted wheats. American 2-row will be the cheapest choice for malted 2-row.

A traditional wit is made with pilsner malt and unmalted (flaked) wheat in roughly a 60/40 ratio. A small percentage of flaked oats can also be incorporated.

Just curious, you are an all-grain brewer, correct?
 
What he told me to use is:

The grain bill is pretty simple: 37.5% malted barley (2-row), 37.5% malted wheat, 12.5% flaked unmalted barley, 12.5% flaked unmalted wheat. You want to target a wort density of about 11-11.5 Plato (which is about 1.044-1.046 SG).

I'm putting this together in beersmith. However my alcohol content is at 4.4% and should be at 4.9%
He's also telling me to mash at 138 which seems rather low.

I am brewing all grain.
