Need carbing formulas please

Starting to bottle. Wanting to know the formulas for the different types of sugars. (built an excel doc.) I don't want to use an app or website.
I know the one for corn sugar weight in grams =
15.195 * Volume of beer in US
gallons * (Desired carbonation level in volumes of CO2 - 3.0378 +
(0.050062 * Fermentation temperature of beer in degrees F) -
(0.00026555 * Fermentation temperature of beer in degrees F^2))

would like to know the formulas for table sugar, dried malt extract, & honey. (in grams weighed)

Thx and sorry if this is posted elsewhere. Did a search and didn't see it.

What i have come up with so far roughly is...

table sugar (100 attenuation)
Table sugar = corn sugar weight * .95

dme (55 attenuation)
DME = corn sugar weight * 1.727273

Honey (84 attenuation)
Honey = corn sugar weight * 1.130952

Would like an accurate formula for each.
 
anyone? last chance and then I will just go with what I got. someone else has to have something like this...
 
Northern Brewer has a calculator on their site. It is at the bottom, click on resources then priming calculator. It lists all the different priming sugars and allows you to select a style of beer or the amount of carbonation you want.

Dont want to use app or website. Would like the formulas. Thx though.
