Kegged beer starts out great, loses head toward end of keg

I'm coming to the end of my second kegged brew and have noticed that both times the beer doesn't retain its head when I get down to the last third to quarter of the keg. The carbonation is fine, though the beer seems a little more watery and "listless" if that makes any sense.

This current brew is an esb that I kegging and carbed at about 40F at 10psi for 2 weeks. I serve it at about 9psi.

Any ideas what might be going on?

Serving pressure lower than the carb pressure for one....
