Gas side ball lock connector leak

Out if nowhere one of my gas side ball lock connectors is leaking. Is there any way to determine how this happened?
 
Do you use something to lubricate the O-rings on your keg posts?
If not, there's the potential to damage an O-ring every time you connect a QD...

I changed and lubed a new o-ring to be safe but the one I took off was newer and still had a bit of lube. I would have put it back on but I did some damage getting it off.
 
Make sure the QD itself is tight, you can put a large screwdriver in the slot on top. There's an internal o-ring that can be swapped out as well.
 
I was able to turn it maybe a 16th of a turn by have not yet tested it. Is this a gasket that is normally lubed?
 
The very thin washer is something like Delrin, and as it's sandwiched between two pieces of plastic there shouldn't be any need for lube...
