I have a three keg set up with a manifold and a single pressure regulator. Two beers with similar gravities under the same PSI (12 lb). Been there for three weeks. One perfectly carbed, on flat (not completely, but largely). Any idea how this could happen?

If everything is working correctly, you would have equal carbination on both kegs at fridge temps.

Sounds like either the manifold isn't passing the gas, or you have a slow leak on the flat keg. Did you hit each keg with high pressure to seal them before purging them back down to force carb levels? It's real easy to have a slow leak in a keg with a unseated seal that's pretty undetectable.
 
I would start with the gas post/disconnect marriage on the flat keg. Perhaps one of the poppets isn't actually opening (likely the post poppet)...
